Office Policy

BILLING INFORMATION:

Patients are required to pay off any balance on their account In 2 weeks.
If longer than 30 days, interest will be added at 8%.

You will be billed for serum in increments of 6 injections at a time. (IE: if you get injections weekly you will be billed every 6 weeks.) You will be rebilled if you go 12 weeks past your scheduled injection frequency.

If you have copay, it is due at time of service for all office visits. Deductibles are also due at time of service. Please check with your insurance company as to what your copay and deductible are. If you do not pay your portion (copay/deductible), your insurance company can choose to put all the charges for the entire visit as your responsibility. Should your insurance change it is your responsibility to notify us and know coverage for your services may change.

Please notify us of any insurance changes so we can correctly bill them. We will also ask to see your driverís license to protect you from identity theft.

If you need to cancel your appointment you need to let us know at least 24 HOURS before your scheduled appointment new patient appointments will be a $50.00 fee, Established patients will be a $25.00 fee. We have an automated appointment reminder if you do not confirm, there will be a verbal phone reminder on your home phone or cell phone you may also be notified by email.

Forms If you need forms filled out for (school, FMLA, or work) there will be a charge of $25.00. If you need copies of records from our office there may be a charge of $.10 per page.

 

INJECTION INFORMATION:

Please be here 40 MINUTES BEFORE CLOSING and WAIT 30 MINUTES after your injection. You can not leave until a nurse or Dr. has checked your arms.

YOU NEED TO MAKE AN APPOINTMENT WITH DR. HUTSON IF: If your current medications are not working; you are ill; if you have had a local reaction to your injection the size of a fifty cent piece or larger; if you have not had your injections for 6 weeks, or if you have a systemic reaction.

Be sure and update the nurses on any changes in meds, health history, etc before receiving your injections.

If you have a long drive to the office, you may call before leaving to verify that Dr. Hutson has not had to leave due to unexpected circumstances. (THIS RARELY HAPPENS)

NO FOOD OR DRINKS are allowed in the office due to other patients having food allergies.

Please do not wear perfumes or fragrances in the office as these affect our allergic patients. Shoes and proper attire are also require in the office and the waiting area.

We do not have a public bathroom in our office, so you need to use the one at the end of the hall BEFORE getting your injection.

PLEASE REFRAIN FROM USING YOUR CELL PHONE IN THE OFFICE/LOBBY. IF YOU NEED TO USE YOUR CELL PHONE, PLEASE STEP INTO THE HALL, BUT STAY WITHIN SIGHT OF US.

It is the parent's responsibility to sign a consent form and to let us know if they are not bringing the minor in for treatment. Consent from legal guardian for others to bring a minor patient to the office for injections is needed-NO EXCEPTIONS. Please request a form to fill out.
